A Team Optimization Project to enhance the care provided to clients living with a chronic disease in rural, First Nation communities by utilizing an interdisciplinary and culturally responsive approach.

Lead Organization: Wellness Wheel Medical Clinic
Focus: Hiring an RN to lead the project and review charts/profiles, holding Chronic Disease Management (CDM) days in different Indigenous partner communities, and creating a more inclusive and culturally responsive approach to care.
