Community Outpatient Services

A Team Optimization Project to elevate primary care by implementing a thorough, interprofessional strategy. This involved enhancing hospital transitions for inpatients and sustaining a team-based dual walk-in and longitudinal care model, all supported by a team with expertise in change management and clinic operations.
